Chicago Hawks Girls Prospects

The Chicago Hawks program believes in long-term athletic development and growth for players on and off the ice. We seek athletes who are willing to commit the time and effort required to achieve personal goals, as well as athletic and academic success. We pride ourselves in having a team for each athlete's skill set. We understand that every athlete’s development and goals are different, and it is our mission to help guide each individual to meet their goals on and off the ice. 

The Chicago Hawks offer a Big Sister/Little Sister Mentor Program which emphasizes the importance of role models and mentorship for our players. Our players support each other at levels and age groups which is what we believe to be the forefront of our program’s success. We instill the Chicago Hawks Core Values in each of our players and focus on Respect & Responsibility.

It is important to show respect for yourself, your teammates, your coaches and your opponents on and off the ice.

RESPONSIBILITY

When you're responsible, your coaches and teammates will count on you to do the things you're supposed to do.

All players must fill out the uniform pack order form below. The uniforms pack includes: Practice Jersey, Practice Sock, Game Shell, Game Socks, Warm-Up Jacket/Pants. Game Jerseys are not included in the uniform pack and will be purchased for an additional fee ($130 for one, $250 for the set).

All first year Hawks and those new to the organization must purchase a set of home and away jerseys. Returning players are not required to purchase new jerseys as long as the jerseys still fit, are in great condition and the player intends on keeping the same number from the previous season(s).

Refunds will only be provided for jersey purchases of players who have not been OFFERED a roster spot within the Chicago Hawks Hockey Club. All jersey number conflicts will continue to be resolved in favor of the longest-tenured player with the most consecutive years in the Club. In the event two or more players with the same number are placed on the same team, the player(s) with less longevity will be required to change their number. Depending on the numbered jerseys the Club has in stock, a player may incur the cost of an additional set of jerseys.

Please allow 6-10 weeks for delivery of jerseys. All other pieces of the uniform pack will be available once teams are selected.

Achieve Sports Medicine is excited to offer the Chicago Hawks/Darien Phantoms Hockey Club baseline concussion testing for the upcoming 2024-2025 season.

Baseline testing is FREE for all Hawks/Phantoms athletes and MANDATORY to complete prior to the start of the season. Your child will NOT be allowed to play until the test is completed.

What is ImPACT Neurocognitive Baseline Testing?

Using the ImPACT Neurocognitive computerized software, your son/daughter will enter a quiet private room, where they will be set up on a computer or tablet and complete a series of "brain games" used to measure their level of symptoms, reaction time, verbal and visual memory, and processing speed.

Baseline testing includes:

• A computerized Baseline ImPACT test used to measure the level of symptoms, reaction time, verbal and visual memory, and processing speed.
• If an injury does occur, a secondary ImPACT test is included free of charge.
• Athletes will be recommended to perform Physical Therapy and further testing based on severity.
