Logo Use Authorization Form

All Chicago Hawks and Darien Phantoms logos are the proprietary right of the Chicago Hawks Hockey Club and may not be reproduced or altered (in any form) without prior consent of the Chicago Hawks Hockey Club. The Chicago Hawks and Darien Phantoms are committed to consistency and uniformity for "branding & identity" of the club. Therefore, no alterations of the original design will be endorsed for entities outside of the Chicago Hawks Hockey Club for apparel or online, print or other forms of communication (posters, merchandise, etc.).

If your team would like to use the Chicago Hawks or Darien Phantoms logos for any reason, we require that you complete the CHHC Logo Authorization Form requesting prior approval. The authorization will NOT give you blanket authority to use the logos. Approvals will be specific to a particular request.

• No one may design their own Chicago Hawks or Darien Phantoms apparel. Any team wishing to do so MUST submit a CHHC Logo Authorization Form.
• No one may profit from the sale of Chicago Hawks and Darien Phantoms apparel.
• All Apparel must conform to the Chicago Hawks Hockey Club Brand Standards. Poor taste, poor quality, or offensive language will not be tolerated.
